• $70 for new / $65 for renewal -must provide proof of unexpired card

First Aid Class

QMAP
Colorado Medical Health Training Inc.

• At least 18 years old. Must bring photo ID
• Finish test and homework at home then walk-in for practical in-person testing every Tuesday at 8am
• Costs $115 / retake the exam $45
• Practical-in-person testing - at the school 6670 South Broadway, Centennial
